COVID-19 Impacts To Our Operations

COVID-19 continues to impact FTRI in-person operations, however we are still processing applications and shipping equipment to new and current clients.

For faster service, we suggest both new and current clients either:

  • Complete the application request form
  • Send an email to: customercare@ftri.org
    • Provide name, address, phone number, date of birth, Florida driver's license number; indicate if you wear hearing aids, or not.
    • Let us know the type of equipment you are interested in (view Products page to see options)
  • Send a fax to: 850/656-6099; include info requested for email
